





Peer Care Coordinators (PCC) / Case Manager 1
provides all care coordination services offered by The Ripple Effect. Peer Care Coordinators helps the person develop a team of health care professionals who work together to ensure that the person’s health needs are being met and that the right care is being delivered in the right place, at the right time, and by the right person. The PCC is knowledgeable about strategies to access to care, provides hands-on support, demonstrates a willingness to arrange and coordinate a complex array of services to meet the need of participants. The PCC is supervised by the Lead Peer Care Coordinator.
Lead Peer Care Coordinator / Case Manager 2
provides primary supervision of the facility and must be on-site at all times during service delivery. The Lead PCC is responsible for the same activities as the PCC but reports directly to the project director. The Lead PCC is responsible for assessing emergencies that may require expelling a participant or contacting medical or law enforcement support, writing incident and project reports and communicating the status of the center with the project director. The lead PCC is also required to perform project presentations, interface with medical, law enforcement, social service providers.
